I know that Norfolk Southern and CSX Transportation both have their own operating rulebooks that they use. Does any other class I railroad or Amtrak use their own modified rulebooks? And, I was wondering if anyone knew where I could find an example of a Norfolk Southern Form 23a, CSX EC-1, or the Track Warrant Equivilent used by the Canadian Pacific and Canadian National.

There is sort of a "master" for which the rule books are derived. The Eastern General Code of Operating Rules, and the Western General Code of Operating Rules. Each railroad bases their rules on these two codes, and then modifies and adjusts the rules to fit their particular operation.
Amtrak uses what is known as NORAC on the NEC. Where they run on CSX and NS they used their books, and everywhere else they use GCOR. Each road has their own Special instructions as well which are addons to GCOR. Amtrak also has their own airbrake and train handling books known as AMT3.
