    I wonder if that's some form of EOT device on that Italian loco, or just a cover?

    I believe that is a cover over the coupler. The use a different coupler system on their passenger trains.

    Here is one type of coupling that it might be. (There are other kinds that are similar.) I believe that it also automatically connects things like air, signal and HEP.
