Here is a locomotive and one car at the My Washington Cog Railway in the White Mountains of New Hampshire. The car is not attached to the locomotive and is pushed on the way up. When coming down the brakeman in the coach keeps enough pressure on the brakes so the car just touches the locomotive. Both the locomotive and car have cog wheels and the locomotive is used for power and breaking while the coach is used only for breaking. They only push one car. The boiler is tilted so it will be level on the way up and down the mountain.
